Treatment course, step by step
- She received 2 cycles of paclitaxel and carboplatin before debulking surgery.
- After surgery she received 3 cycles of albumin-bound paclitaxel and carboplatin.
- After progression, toripalimab was added and treatment later changed to gemcitabine, bevacizumab, and toripalimab.

What happened, in summary
A 44-year-old woman presented in late 2021 with abdominal swelling, reflux, and pain. Imaging showed large masses involving both ovaries, extensive disease across the abdomen, and ascites. A biopsy confirmed ovarian clear cell carcinoma. The cancer was classified as Stage III.
She began paclitaxel and carboplatin chemotherapy, then underwent extensive debulking surgery in March 2022. Cancer remained on several abdominal surfaces, and she received additional chemotherapy after surgery.
Within 3 months, imaging showed new liver lesions and the cancer was considered resistant to the original platinum-based treatment. Molecular testing found HRD positivity, an ARID1A mutation, and high PD-L1 expression. Her treatment was adjusted several times, including toripalimab immunotherapy, gemcitabine, and bevacizumab. Bevacizumab was later stopped after another blood clot developed.
She continued gemcitabine plus toripalimab and achieved a strong response, followed by a complete response during maintenance toripalimab. She completed 18 cycles of maintenance immunotherapy in May 2024 and then continued oral anlotinib. At the latest follow-up, imaging and tumor markers remained normal.
