What happened, in summary

A 77-year-old man presented with two months of persistent swelling and pain below the breastbone. Imaging showed multiple lesions in both lungs and multiple liver metastases.

A liver biopsy showed small cell carcinoma with TTF-1 expression, supporting a lung origin. Gastroscopy also identified a small depressed lesion in the stomach, and biopsy showed a neuroendocrine pattern consistent with metastatic small cell lung cancer. These findings established widespread disease involving the liver and stomach.

He did not receive anticancer treatment and died several months after diagnosis.
