A 64-year-old woman presented in August 2019 with cough, coughing up blood and low-back pain. Imaging showed a left lower-lobe lung mass, cancer in both lungs and bone metastases. Biopsy confirmed lung adenocarcinoma, and molecular testing found an EGFR L858R mutation together with a TP53 mutation. She was diagnosed with Stage IV disease.
She started icotinib in September 2019 and achieved a partial response. In April 2020, the lung tumor enlarged and blood testing detected an acquired EGFR T790M mutation. She switched to osimertinib and again achieved a partial response.
By May 2021, the cancer had progressed and testing identified a new EGFR L718Q resistance mutation along with L858R and TP53. She started dacomitinib in June 2021. The disease was stable after one month, but progression was documented again in September 2021. She was alive at the last follow-up.
