A 74-year-old woman developed swelling of her left arm before imaging revealed stomach-wall thickening, ascites and extensive lymph-node abnormalities. Endoscopy and biopsy diagnosed gastric adenocarcinoma with signet-ring and poorly differentiated features. PET imaging showed widespread disease, including unusual diffuse uptake throughout the thyroid.
During hospitalization, she developed painful swelling of the neck and chest. Fine-needle aspiration of the thyroid confirmed metastatic gastric carcinoma, and a skin biopsy from the chest also showed metastatic cancer in small blood vessels and lymphatics. These findings established Stage IV gastric cancer with confirmed spread to the thyroid and skin, while imaging also suggested extensive nodal disease.
She received paclitaxel chemotherapy once, but severe cytopenia developed and her performance status declined substantially. Worsening airway edema was treated with glucocorticoids, and further care was supportive. She died about 1 month after hospitalization.
