A 69-year-old woman presented with a lump in the upper inner area of her left breast. Ultrasound and MRI showed a suspicious lesion measuring about 1.5 to 1.6 cm, and needle sampling confirmed malignant cells. She underwent breast-conserving surgery with sentinel lymph node biopsy.
Pathology confirmed tall cell carcinoma with reversed polarity, a rare breast cancer subtype. The final tumor measured 2.5 cm, and all 5 sentinel lymph nodes were negative for cancer. Biomarker testing showed that the tumor was negative for estrogen receptor, progesterone receptor, and HER2. Molecular testing identified IDH2 and PIK3CA mutations, which supported the diagnosis.
She did not receive chemotherapy or radiation after surgery. At 13 months after surgery, she was alive and disease-free.
